Primary care is essential to ensuring postpartum access to longitudinal postpartum care. After the obstetric postpartum visit, women usually stop seeing the obstetric team that provides care during pregnancy. Yet, pregnancy can increase the risk for or worsen conditions such as high blood pressure, diabetes, and depression, among others.

CONTINUUM’s Primary Care Associations (PCAs) [the Ohio Association of Community Health Centers (OACHC), La Asociación de Salud Primaria de Puerto Rico, inc. (ASPPR), and the District of Columbia Primary Care Association (DCPCA)] support Federally Qualified Health Centers (FQHCs) who provide a range of primary care and women’s health services, with a special focus on communities with limited healthcare access. FQHCs also have experience in working closely with community-based organizations that play a critical role in serving postpartum women.

CONTINUUM’s partnership with its regional PCAs is critical to its central mission of promoting access to holistic longitudinal postpartum care, especially within communities with limited healthcare access.
