Primary care is essential to ensuring postpartum access to longitudinal postpartum care. After the obstetric postpartum visit, women usually stop seeing the obstetric team that provides care during pregnancy. Yet, pregnancy can increase the risk for or worsen conditions such as high blood pressure, diabetes, and depression, among others. CONTINUUM’s Primary Care Associations (PCAs) [the
